Hi, I'm a veterinarian, I'm sorry to hear that your dog is so sick.
This sounds suggestive of a gastric ulcer or other gastro-intestinal bleeding. Is there any chance your dog could have got into any anti-inflammatories like ibuprofen (advil), aleve, or is your dog regularly on an anti inflammatory like rimadyl or metacam?
If your vet hasn't done so already, I would suggest you ask your vet about running a chem panel to check out kidneys and liver, and a CBC to see if there is any bleeding internally, and do an abdominal xray, its time to get aggressive about investigating.
I would suggest you take your dog to the vet tonight, 2 days is too long to be totally refusing food, if he isn't drinking he will be getting badly dehydrated, and kidney failure needs to be ruled out.
I can't give you any idea of prognosis, nor can anyone over the internet, as we don't have a diagnosis.

I'm not a vet and I do not know your dog, but my dog is going thru the same thing right now.
1. Do not wait days to see the vet when your dog won't eat or drink, it is serious.
2. Make SURE you have an excellent vet who is up to date on things & who cares about your dog, AND one who always has someone on call for emergencies.
3. Make sure you have & know the plan for emergencies with your vet, have ER phone #s.
4. Have a syringe (no needle) to put water in your dog by mouth if needed.
5. Things your dog may eat: scrambled eggs, mashed potatoes, baked sweet potatoes, boiled chicken
breast, rice, oatmeal, yogurt, cottage cheese. I mix chicken & rice. Ensure vanilla.
After he's been vomiting dogs /people don't want to eat for awhile.
But try water in small amounts. I offer small meals, 1-2 Tbls at a time. I give runny oatmeal first & he likes that, then eggs, then shredded chicken & rice.
I just cook everything now, store bought food won't stay down.
Good luck.
